Indiglow LED T8 Lamps Recalled due to Injury Hazard

  • Issue: The recalled lamps can overheat causing the glass tube to fall and strike those standing nearby, posing an injury hazard.
  • What to do: Immediately stop using the recalled product and contact Ushio America for instructions on how to return the product for a full store credit or a free replacement lamp.

Product description

This voluntary recall involves Ushio America’s Indiglow LED T8 Black Light lamps (Item #3000677), with a 1.2 metre (4 foot) tube. The lamps are direct drop-in replacements for 30W and 32W T8 fluorescent tubes. The firm name USHIO and brand name Indiglow are on the lamp. The manufacturing date code is imprinted on the silver aluminum end cap of the lamp indicated by 16xx or 17xx.  

Hazard identified

The recalled lamps can overheat causing the glass tube to fall and strike those standing nearby, posing an injury hazard. As of July 2, 2020, the company has received one reported incident in Canada of a lamp falling, no injuries were reported.

Number sold

Approximately 75 units of the affected product were sold in Canada.

Time period sold

The affected products were sold from June, 2017 to February, 2018.
